Exam Proctor Form

​The Exam Proctor form is a convenient way for faculty to submit proctor instructions to the testing center for PAPER or ONLINE exams.

The form has been recently updated so that you can now submit instructions for a Single Student or for Multiple Students.

Please remind your students that in order to test, they need to:

  • provide their instructor's last name
  • present a valid PHOTO ID
  • know the Testing Center's open hours (each center's hours can be accessed from the Testing Cen​ter Home Page)

FORM INSTRUCTIONS:

  • SUBMIT FOR ONE STUDENT: Choose “Single Student" from the dropdown. Enter the DMACC Student ID (starts with 900 or 901) and click on Lookup ID; the student information will populate. Please click on the COURSE dropdown to select your course from the list
  • SUBMIT FOR MULTIPLE STUDENTS: Choose “Multiple Students" from the dropdown. Be sure to attach a class roster or a file with the names of the students who will test in the testing center.

PLEASE NOTE:

  • When entering your phone number, please do not let it autofill from your PC or your student's name will be replaced with your name. Type in your phone number instead.
  • PAPER exams will route to respective testing centers to be printed
    • Please remember to ATTACH the exam at the bottom of form
    • Please be sure to select ALL the items the student is allowed to have in the testing room
    • Additional instructions for the Testing Center can be typed in the “Proctor Exam Instructions" field
  • ONLINE exams will prompt you for an Exam Type and an Online Password
    • Please remember to open the online test so it is available when the student arrives
    • Please be sure to select ALL the items the student is allowed to have in the testing room

  • Upon submitting the exam information, you will get an email stating that the Exam Proctor Form has been submitted
    • The designated Testing Center will receive the exam information
    • The student will receive an email stating that the exam has been submitted to the Testing Center (they won't receive passwords and proctor exam instructions; they only receive the due date and what they are allowed to have for the test)
  • Multiple Student submissions where instructors attach a roster instead of using student ID numbers will not send an automated email to students to notify them that a proctor form has been sent to the testing center. We hope to improve this function in a future enhancement of the form.

Paper tests being sent to the Ankeny Testing Center will be printed by the staff in the Ankeny Testing Center. Please allow up to 24 hours for the test to be copied depending on the time the test is submitted. It is the instructor's responsibility to pick up completed tests from the testing center. For follow-up or questions, please call the Ankeny Testing Center at 515-964-6595.

Paper tests being sent to the Urban Testing Center will be printed by the staff in the Urban Testing Center. Please allow up to 24 hours for the test to be copied depending on the time the test is submitted. It is the instructor's responsibility to pick up completed tests from the testing center. For follow-up or questions, please call the Urban Testing Center at 515-697-7809.

Paper tests being sent to the Boone Testing Center will be printed by the staff in the Boone Testing Center. Please allow up to 24 hours for the test to be copied depending on the time the test is submitted. Instructors may pick up completed tests from their mailboxes.
